Health Promotion Projects in Primary Care
Health Promotion might be a bit of a mystery for some people, but the ideals of Health Promotion have been part of many of our jobs for a long time.

Health Promotion aims to empower people, families and communitites to increase control over their own health through a range of tools including creating systems and environments that are supportive to healthy change. At Capital PHO we take this very seriously and have been providing training, funding and support for our Medical Practices to carry out Health Promotion projects with their enrolled populations for over 3 years.

See the attached download for some of the great projects currently underway in our Medical Practices.

If you have questions or would like more information, please contact Rachel Harrison on 04 978 4366 or through the contact us page on this website.

Capital PHO part of Diabetes Education Research
NZ Group-based Self-Management Education for Patients/Whanau with Type 2 Diabetes
International evidence demonstrates group-based self-management education facilitates improved glucose control, better understanding of diabetes and improved quality of life. A New Zealand research team is developing an education programme that meets the specific needs of our population. This will be deliverable in primary care settings and includes distinct Maori and Pacific components which will be developed in partnership with those communities.

Liz Dutton, Diabetes Nurse Specialist for Capital PHO, is a member of the research team which includes representatives from Secondary care, PHOs, Maori and Pacific stakeholders and University. She will be one of the facilitators of the programme to be piloted, and be involved in its revision. She is also involved in designing the resource toolkit which will be used by the facilitators in the group sessions.
