Outreach Nursing
Outreach nurses help people who have difficulty accessing their GP or practice nurse for reasons such as cost, transport, language, and/or cultural barriers. Outreach nurses can provide assistance for GP visits, transport to appointments and medications. They also work with other agencies such as Work and Income and CYFS to ensure other social needs are being met.

Our Outreach Nurses are a mobile service, and they can meet with you where you are most comfortable to discuss your health issues. They can help you with:
  • Cervical smears
  • Cardiovascular risk assessments
    (heart checks)
  • Mammograms
  • Diabetes annual reviews and testing
  • Blood tests
  • Immunisations
  • Health checks
Who can access this service?

All Pacific patients who belong to one of Capital PHO’s 32 practices (or people who do not yet have a GP).

How do I access this service?

Primary Solutions
What is this service?
Primary Solutions is a programme which aims to improve access to health and social services to support mental wellbeing for those with a mild to moderate mental health issue (e.g. stress, anxiety, depression, and dealing with grief, loss and change). The Primary Solutions Coordinators can:
• Assist you to identify any issues that may impact on your mental wellbeing
• Provide information about services currently provided within your local area
• Support you to access services relevant to your needs (some services can be funded)
• Work with you to plan a flexible package of care to meet your individual needs.

Who can access this service?
You can access this service if:
• you are enrolled or intend to be enrolled with a Capital PHO practice
• you have a mild to moderate mental health issue.
AND if you are
• Between 10 and 24 years of age, or
• Maori, or
• A Pacific Islander, or
• Have or are eligible for a Community Services Card.
If you have any doubt about your eligibility talk to your Doctor, Nurse or a Coordinator and ask for advice about what is available in your community.

Diabetes Education

Diabetes is increasing at a rapid rate in New Zealand. Half the people who have it are unaware, as they have not been checked for it.

Ask your doctor or practice nurse for a Diabetes check if:
• You are Maori, Pacific, Asian or Middle Eastern over 30 years
• You are European over 40 years
• You are overweight
• You have a family history of diabetes

If you have diabetes it is important that you have a yearly check with your doctor or practice nurse to make sure any problems are detected quickly. Your Diabetes Annual Review is a free service.

If you feel that you are having difficulty managing it, or you do not understand why you are unable to keep you blood sugar in the target range, you should discuss it with your doctor. They may think you would benefit from a teaching session with your practice nurse, or Liz Dutton, our Diabetes Nurse Specialist. Liz is able to help you gain better control of your diabetes

Who can access this service?

All people who belong to one of Capital PHO’s 32 practices (or people who do not yet have a GP) and have diabetes.

How do I access this service?
You can be referred by your GP or practice nurse.

Liz also supports practice nurses and GPs in their management of people with diabetes and can offer education and advice to them via telephone; and also by education sessions. If you are a practice nurse or GP needing advice or have ongoing education needs, please contact Liz Dutton on 978 4364.

Community Dietician Service
The Community Dieticians Service provides a community based clinical dietetic service focused on people with long term conditions i.e. poorly controlled diabetes and high risk of cardiovascular disease.

The Community Dietician Service can help you with:
  • Individual Referrals
  • Referrals to the dietitian’s service can be made by primary care providers. Referral forms are available on Medtech. Before referring anyone to this service they must have had basic nutrition advice provided by their primary care provider and deemed to benefit from further in depth advice.
  • Group Education Sessions
  • Louise would be happy to attend community groups, exercise groups or already established self management groups to run interactive nutrition education sessions.
  • Health Provider Education
  • Nutrition education sessions are available for primary care staff who would like to work more effectively with patients with long term conditions. Louise can come to your practice to deliver a fun, interactive, practical hands-on session about food.
Who can access this service?

Priority groups for the service include Maori, Pacific, migrants, refugees and people on a low income. All other referrals can continue to be sent to the hospital dietitian services.

CarePlus

Care plus is a programme that provides free and reduced cost visits to your GP and practice nurse, and supports you to develop a Wellness Plan to help understand and manage your health.

What do I get out of it?
1. A free initial assessment with your GP or practice nurse to develop your Wellness Plan
2. A reduced cost visit to your GP or practice nurse after 3 months to follow up on your Wellness Plan
3. A free visit 6 months after your first visit
4. A reduced cost visit to your GP or nurse after 9 months to follow up and set goals for next year

How can I get onto Care Plus?
If you meet any of these criteria ask your practice about being on Care Plus:
• Have two or more long-term conditions, e.g. diabetes, high blood pressure, asthma, or
• Have had six or more visits to your doctor or the emergency department at the hospital in the past six months, or
• Have had two acute non-surgical admissions in the past 12 months, or
• Have a terminal illness, or
• Are waiting on a waiting list for an operation.
So if you meet any of these criteria, next time you see your GP or Practice Nurse simply ask “Can I be on Care Plus?” or contact your practice nurse for more information.

Immunisation Co-ordinaton Service
Immunisation can protect people against harmful infections, which can cause serious complications, including death. It is one of the most effective, and cost-effective medical interventions to prevent disease.

Immunisation uses the body’s natural defence mechanism, the immune response, to build resistance to specific infections. When an immunised person comes in contact with that disease in the future, their immune system will respond to prevent them developing the disease.

The National Immunisation Schedule is the series of vaccines that are offered free to babies, children, adolescents and adults. Your Practice Nurse or Doctor can explain more about what each immunisation is for and when you or your child should be immunised.

Our Immunisation Co-ordinator, Fiona Johnston, supports General Practices with their immunisation programmes, assists in maintaining nursing vaccination standards, provides assessment for non-medical vaccinators and assists in the delivery of continuing education. All health professionals who belong to one of Capital PHO’s 32 practices can access this service.

Active Families

Active Families is a programme which assists children and their families into healthier, more active lifestyles. It encourages physical activity and teaches healthy eating habits to help them maintain a healthy weight and prevent or control medical conditions.

Who can be referred?
Children and young people between the ages of 5 to 18 years old can be referred by a health professional. Families are also able to self-refer onto the programme.

What is involved in the programme?
• Monthly home visits to encourage and educate the family to be more active and adopt a healthier lifestyle.
• Weekly Activity Sessions which the entire family can attend. Families have fun participating in a wide range of sports, games and activities, including healthy cooking sessions.

Pasefika Healthy Lifestyle Programme

Our FREE Pasefika Healthy Lifestyle Programme Exercise Classes are held 3 times a week at Pacific churches around Wellington.

They’re an hour of fun Aerobic exercise with a warm up and cool down led by our experienced fitness instructor, Tone Tipene Te Ahuru.

They're a great way to get active in your community for free, and everyone is welcome - you don't need to be Pacific to come along to the classes! Bring along a towel, some water, and your friends and family!

Healthy Lifestyles
Capital PHO provides support and resources for individuals, families and communities to access activities that help keep them healthy.Some services we provide include:

Pool vouchers programme
Working alongside Wellington City Council and other Wellington PHOs to increase access to pool-based exercise.

Workshops for Parents
Delivered by Margaret Ross, Adolescent Behaviour Consultant. Come to a 2-hour interactive workshop and go home with negotiation and problem solving skills to use with your teenagers.

Help with quitting smoking
All our doctors can provide information on quitting smoking, and access to subsidised nicotine patches, gum and lozenges. Extra support for smoking cessation includes:

Maori Community Health Co-ordinator

Linking with Maori communities to increase awareness of health information, and linking Maori and their whanau into activities, health provides and services provided by the wider community.

Some services we provide include:
• Maori Outreach Nurse, Robina Wichman can visit patients in their home if they are having difficulty accessing their GP for reason such as cost, transport, language barriers etc.
• Low-cost GP service through Ngawari Hauora Medical Centre. Offers a FREE Doctors service to children under 6 years and between $10 - $15 (according to your age) for over 6 years.

Who can access this service?
All Maori people who belong to one of Capital PHO’s 32 practices (or people who do not yet have a GP).

Pacific Community Health Co-ordinator

Working with our Pacific communities to increase awareness of health information, and linking Pacific people and their fanau into activities, health provides and services provided by the wider community.

Some services we provide include:
• Our Pacific Outreach Nurse, Emma Naoupu Pritchard (Samoan) can visit Pacific patients in their home if they are having difficulty accessing their GP for reason such as cost, transport, language barriers etc.
• Our FREE Pasefika Healthy Lifestyle Exercise Classes are held three times a week. See the article above for more information.

Who can access this service?
All Pacific Island peoples who belong to one of Capital PHO’s 32 practices (or people who do not yet have a GP).

Interpreting service for Capital PHO patients
Capital PHO works with Interpreting Wellington and Language Line to provide interpreting services for people who would prefer to speak in their own language when talking to our doctors or nurses. Using both providers allows patients the choice between face-to-face or telephone interpreting. More than 40 languages are currently available and this service is free to all people enrolled with Capital PHO.
